Staying true to its name, the complete song is an unabashed display, immersing the duo's allure in a dynamic pop arrangement featuring Eastern-influenced strings. Madonna contributes fiery ad-libs, including lines like "Do you know how to spell my name?/ B I T C H/ Go fuck yourself/ You heard me, bitch."

However, "Vulgar" isn't gratuitously explicit; instead, the artists are conveying a message of empowerment and insisting on respect. In February, Madonna introduced Sam Smith's performance of "Unholy" with Kim Petras at the 2023 Grammy Awards, triggering criticism from conservative figures. Addressing the Grammys crowd, Madonna posed the question, "Are you ready for some controversy?" She continued, "If they call you shocking, scandalous, troublesome, problematic, provocative, or dangerous, you are definitely onto something."
